Salary: £41,808 – £45,021
Hours: 36 hours per week
Golden Handshake: £5,000 (after satisfactory probation)
Retention Payment: £3,000 after one year for eligible roles
Overview:
We are seeking a compassionate Social Worker to join London Borough of Brent’s dedicated Adult Social Care team. This role involves carrying out needs-led assessments, managing cases & reviewing & co-ordinating multi-service care packages. You will work closely with service users, carers, and multi-disciplinary teams to deliver high-quality services that meet residents’ needs. Key Responsibilities:
- Conduct needs-led assessments of adults in line with the Community Care Act.
- Support the unit in achieving performance targets and contribute to its continuous improvement.
- To formulate appropriate care plans and negotiate care packages with service users and service providers. This will involve formulating multi-service care packages, and reviewing on a regular basis.
- To carry out all duties in accordance with the council’s policy and procedures.
- Manage cases including safeguarding adults and Deprivation of Liberty (DoLS).
- Support the team in meeting performance targets and driving continuous improvement.
- Work collaboratively with internal and external partners to ensure seamless service delivery.
About You:
- Relevant social work qualification with current Social Work England registration.
- Demonstrable experience in adult social care, mental health, or disability services.
- Ability to work under pressure, prioritising and managing your caseload efficiently.
